New student enrollment for DeQuincy Middle School will be held July 28 -30 from 9 a.m to 12 noon.
The following documents are needed to complete the application process: Birth certificate, Immunization records, Social Security card, legal documents if applicable, photo ID of parent/guardian, and report card from previous year.
Also needed: Two current utility bills of the homeowner with service address listed. One must be the electric bill. Other utility statements may be gas, water or cable bill. Lease/rent agreements are also accepted provided parent’s name is listed on lease. If electric bill is not in parent/guardian name, you will need a notarized statement stating utilities are in someone else’s name
If you cannot obtain the required documents, go to the child welfare and attendance, located at 2423 6th St, Lake Charles, to obtain a permit to register.
